Student Acknowledgement
I acknowledge receiving, reading, and understanding the Panther Athletics Handbook for the 2025-2026 athletic year. I also acknowledge and understand this handbook is available on the Athletics webpage of the Oakhill Day School Athletics website, and may also be requested from the Athletic Director or Front Office.
I acknowledge that participation in the athletic program at Oakhill Day School is a privilege and not a right. As such, I understand that it is my responsibility to uphold and adhere to the expectations and guidelines provided in this handbook and the School Handbook.
I acknowledge and understand that my actions have meaning and that there will be consequences for not following and/or breaking school policies. I know that if I have any questions about the content, meaning, or interpretation of the handbooks, I should contact the Athletic Director or the Head of School.
Parent Acknowledgement
Further, as the parent(s) or guardian(s) of the above student-athlete, I (we) also acknowledge receiving, reading, and understanding the Panther Athletics Handbook and understand what is expected of my student. I also acknowledge and understand my (our) responsibility in positively supporting, encouraging, and following the expectations contained herein.
By agreeing to the guidelines outlined in this handbook, I (we) give full permission for my (our) child to participate in the athletic programs and sports offerings of Oakhill Day School. I (we) affirm that my (our) child is physically, emotionally, and mentally able and willing to participate in any athletic activities or programs in which they are enrolled or choose to participate. I (we) understand that Oakhill does not require a sports physical until 6th grade, and by granting this permission, I (we) accept full responsibility for my (our) child’s readiness and ability to safely engage in these activities.
Note: Only one parent signature is required if returning the paper form located at the back of the handbook. Otherwise, by clicking “I agree,” you are agreeing to the guidelines outlined in the handbook.
