Parental Agreement Statement

I, the undersigned parent/guardian, grant permission for my child to participate fully in Oakhill Fully Charged's Summer Camp Program at Oakhill. This includes activities, field trips, use of the pool, and photography for promotional purposes, among other activities.

I understand that my child's involvement will be in accordance with their age and the camp's programming. I trust the camp organizers to ensure their safety during various activities and field trips.

I authorize my child to attend the pool under supervised conditions, with appropriate safety measures in place.

I also grant permission for Oakhill Fully Charged to use photographs of my child for promotional materials.

By signing below, I confirm my consent for my child's participation in the camp activities, including but not limited to those mentioned above.
